Rates and Insurance

We offer self pay rate, which is due at time of service for the following:

• Individual therapy 
• Couples therapy 
• Family therapy 

Services may be covered in full or in part by your health insurance plan. Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician

Please see below of insurance plans accepted, and we will assist with checking your benefits prior to your first visit.

Commercial Plans:

• Blue Cross Blue Shield (Blue Choice PPO Preferred, Blue Choice Options, Professional Blue Choice PPO, and Preferred Provider Organization)
• Cigna 
• United Healthcare
• Aetna

Medicaid Plans:

• Blue Cross Community Health Plan (includes MMAI)
• Aetna Better Health (includes MMAI)
• Meridian (includes Youthcare)
• CountyCare
• Molina (HealthChoice Illinois and MMAI)

If you are requesting a consultation prior to setting up services with a therapist. There is a $25 fee. Consultations are limited to 15 minutes. 

Payment (co-payments, deductibles, and co-insurance) is due at the beginning of each session.   

Cash, Check, Zelle, HSA/FSA cards, and all major credit cards accepted for payment.

We will submit claims to your insurance carrier on your behalf. However, please remember that your insurance coverage is a contract between you and your insurance company. While we will work with you and your insurance company to help expedite claim payments, our primary relationship is with you and not your insurance carrier.

If your insurance company denies all or part of your claim, you are responsible for payment of the balance. Since you are the customer of your insurance company and pay the premiums (either directly or indirectly), your insurance company is ultimately responsible to you and should address any questions or concerns you may have about your coverage.

If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you may be required to pay a no show/late-cancelation fee.
